The Clubhouse/Gym is open from 5 am-12 am. Pool hours are from 9 am-12 am on Mon-Fri and 5 am-12 am on
Sat-Sun. The Laundry Room open is 24 hours every day. Fobs are required for all amenities. Quiet hours are from
10 pm to 8 am. Quiet hours apply to all apartments as well as basketball courts, pool/hot tub, and other general
areas/amenities.

When You First Move In
On Resident Portal, you must log in and do the move-in inspection. On this inspection, you must report damages
in your apartment within 72 hours of your move-in. Damages that are not reported after 72 hours will be
considered yours at move-out. You will be able to complete a move-in inspection on your resident portal. This
will be available within 24 hours of your move-in. You will be able to take pictures of the condition of your
apartment and create work orders within this inspection.
Rent & Utilities
Rent and utility payments are due on the 1st of each month. Any unpaid balance will receive a late fee ($75) at
12:00 am on the 6th of each month. We will not waive any late fees for any reason (this includes getting paid
late or auto payments not working). While only checks and money orders are accepted in the office, e-check
payments can be made online. We cover the cost of e-checks, but The Crestwood does not cover credit card
service fees and vary based on the credit card company.
Mail and Packages
Your resident address is 1800 N State St., Apt #__, Provo, UT, 84604. Make sure you don’t add your bedroom
letter to this address, it will confuse the mail carriers and they will end up misplacing your package/mail. Just
add the Unit number and nothing else for the apartment number. EX: 1800 N State St., Apt #101, Provo, UT,
84604. In order to receive mail and packages, you MUST have your apartment number listed. You can contact
the post office to redirect your mail.
Apartment Conditions
Reporting damages in your apartment is required within 72 hours of move-in, or they will be assumed yours at
move-out. You should receive a Move-In Condition Form via email, in which damages will be reported. If you do
not receive such a form, please contact the office immediately so we can get the form to you before the 72-hour
deadline.
Parking
All residents need to be familiar with the parking rules. All residents with vehicles must obtain a Parking Permit
before they can park at The Crestwood overnight. In this packet there will be a step by step on how to enroll
digitally and receive digital parking permits on move in day. Visitors must obtain a guest pass via the UPE
website or PARKUT app. Parking is monitored 12 am -8 am by UPE. Crestwood is not responsible for parking
enforcement or violations and does not receive any compensation. If you are in violation, you must contact
UPE at (801) 377-8200. If you feel you have been unlawfully booted or towed, you may contest this operation as
provided in section 6.01.080 of the Provo City Ordinance. You may also appeal a parking enforcement fine
through University Parking Enforcement’s website. If you have a Motorcycle, you don’t need a parking pass. You
will have to park it in the same parking spot as your car. It has to be parked in the back part of the parking
spot/closer to the curb than your car. If you have multiple cars, please contact the office, and we will be able
to help!
Internet
The Crestwood provides house internet through Veracity. The internet was recently upgraded to gigabit speeds,
and the router is attached to the wall in your living room with the password labeled on it. You will not need to
bring a router to your apartment. If you are having difficulty connecting, we encourage you to call Veracity’s
help centers at (801) 456-1000. The office cannot help with internet issues, so please refer to the Veracity
support line.
Cleaning Checks
Cleaning checks will be performed quarterly, and residents will be given a one-week notice. During the week
before cleaning checks, management will post each bedroom’s chore responsibilities on your door as well as
send you an email with the chore list attached. It is your responsibility to contact the office if you do not receive
this sheet before cleaning checks are performed. Management is not responsible if sheets are removed from
doors or are not seen by residents in time for cleaning checks. If cleaning checks are failed, each resident who
fails (based on their assigned chore list) will be charged a $30 re-check fee. If the apartment is still not clean
after re-checks, we will send in a cleaning crew to clean the failed areas at $40/hour charged to the resident.
Quiet hours/rules
Quiet hours are from 10 pm-8 am every day. If you have issues with your roommates with noise during quiet
hours, you need to talk to your roommates and see if you can solve it between yourselfs. If you are still having
issues after talking to them, then contact the office. If a unit around you is being loud, please take video of the
noise/have a timestamp of the video and then report it to the office or contact the police with the non-
emergency number: 801) 852-6210
Bike License
Unfortunately, bike theft is a reality in Provo, Utah. A good way to protect your bike is to register it with the BYU
Police. You can do this by going to https://security.byu.edu/parking-services and clicking on the “register your
bike” icon on the website. This registration is FREE!!!
Maintenance
Anything wrong or broken with your room/apartment must be reported. To fill out a work order, go into the
resident portal and click on Maintenance. Once you click that, you can make a work order! If you are having
problems with making a work order then text the office. When submitting a work order, be as detailed as
possible to help our maintenance team diagnose any issues in your apartment in a timely manner. Work orders
are dealt with in the order that they come in. So if someone puts a work order before you, we will work on that
first work order first. The only time someone else’s work orders are prioritized is when it’s an emergency.
Animals/ESA6
If you have/are planning on getting a ESA, you must contact the office first. Once you contact us, we will send
you the ESA documents that we require all ESA to have. If you are seen or have been found with a animal and is
not an ESA, we will contact you with the documentation that is needed to have an ESA at Crestwood. If you
refuse to fill out the ESA documentation, then we will give you a “Comply or Vacate” statement on your door.
For any ESAs that are documented, you must follow the ESA rules (we will provide you with that with the ESA
docs). Any unpicked-up poop by an ESA on the property will be fined to the ESA owner. I want to change the title
to this section to Animals/ESA/Allergies I want to add this to the end of the current paragraph: If you have
allergies and can’t live with ESAs then you will need to provide a doctor’s note. Once you get that doctor’s note
depending on who submitted the paperwork first, we will go off that order on who will switch rooms. It is first
come first serve. We are only following federal fair housing laws so I do apologize if this causes any
inconveniences.

● Pay any remaining balance on your account.
● Received Keys: Front Door Key, Bedroom Key, Fob, and Mail Key (One Mail Key per
apartment)
● Find your Apartment and Bedrooms. Look at the layout page on the front door to
find you room. If the Letters are incorrect on the bedroom doors then make a work
order to fix it. If you don’t know which room you are assigned, just look at your lease.
The letters are next to the unit number on the lease. Letters might be wrong on
the Physical Door. The diagram on the front door is 100% what you should go off of
for room assignments. If you are wanting to switch rooms with a roommate then
you MUST CONTACT the office so we can fix your lease. You are LEGALLY
RESPONSIBLE for the room on YOUR LEASE.
● Make the Parking Permit before the end of the day! Instructions are in the
packet!
● Complete your move in inspection on your portal within 72 hours. Damages that
are reported after 72 hours will be considered yours at move-out.
● Change your address. Don’t forget to include your apartment number! Example:
1800 N State St. Apt, #__. Provo, UT 84604-1308, United States. Please return any
mail that is not yours to the sender.
● Download the Resident Portal App, this is the app you can pay your rent and
make maintenance requests with. The Logo is a red “RP”.

✔ How to Make a Parking Permit ✔

University Parking Enforcement has changed from the old parking stickers to the new Digital Parking Permit Program.
This initiative aims to improve parking efficiency, increase convenience, and ensure fair access to parking spaces for
residents and visitors. Here are the critical details about the Digital Parking Permit Program and the accompanying
parking rules:
Digital Parking Permit Agenda: Physical parking permits will be replaced by digital permits.
Each resident will sign up for an account and receive a unique digital permit associated with
their vehicle(s). The digital permit will be linked to your address and vehicle’s license plate
number and will be valid for the duration of your lease agreement. Displaying a physical
permit on your vehicle will no longer be required.
Registration Process:
1. In order to obtain your digital parking permit, you will need to register your vehicle
and sign up for an account at www.utahparkpass.com or Download the PARKUT
app (the app is more user-friendly). The step-by-step is done with the App.
2. Make an account
a. Click on Create account
b. Click on Resident
c. Fill out the Information to make the account
d. Click next
3. Verify your email
a. Go to your email and click on the verify email
b. Click on the link to verify your email
c. Go back to the app
4. Permit Address
a. Enter the address: 1800 N State St
b. Enter Apt #
c. Click on your apartment
5. Make a Security Question
a. Choose which security question you want
b. Put in your answer
6. Document upload
a. You will need to upload your driver’s license (make sure it’s a valid Drivers
Licence & it has the resident’s name)
b. You will need to upload the resident’s Rental Agreement; you can find this
in your Resident Portal App.
i. Open the Resident Portal App
ii. Log in
iii. Click on “View Documents”
iv. Click on “Other Documents”
v. Click on your Current Lease
vi. Take a Screenshot of the first page of the lease (make sure the
residents name and the dates of the current lease are in the
screen shot)
c. Click on Upload File (UPE will have to approve the Documents, even
though the documents aren’t approve, you can still move forward with
making the permit)
7. Make the permit (It will take you to a page that says “Welcome”)
a. Click on Add Permit
b. Click on the date that the permit will start (just make it for today)
c. Make sure it says,
“I will be parking at: Crestwood Resident – 1800 N
State ST,….)
d. Click on Crestwood Resident Permit
e. Click on “Add Vehicle” (this will take you to a vehicles list, there shouldn’t
be any vehicles to click on, so you will have to add one)
f. Click on the “+ Add”
, on the top right
g. Enter your Car’s Information
i. Nickname
ii. Plate
iii. State
iv. Make
v. Color
vi. Model
h. Enter your Registration Details
i. First Name
ii. Last Name
iii. Address
iv. City
v. Zip
i. It will most likely ask if you want to use the Corrected address
i. Click on the corrected address
ii. Click on Continue
j. Click on your car license Plate that you just made

l. Review the Information (Permit, Price, Permit Date, and Vehicle)
i. Make sure it is all correct (click on cancel or back if it is not
correct)
m. Click on Finish (it should say “Permit has been purchased”)
8. Text the UPE to approve the Pending Document and to approve the Permit:

a. Put in the visitor Code (The resident can get this by going to their app and
clicking on “Get Visitor Code”)
4. Scroll down and Click on Crestwood Visitor Permit
5. Click on Add Vehicle
6. Click on “+Add” (top right corner)
7. Click on My Vehicles
8. Fill in your vehicle information
a. Nickname
b. Plate
c. State
d. Make
e. Color
f. Model
9. Enter your Registration Details
a. First Name
b. Last Name
c. Address
d. City
e. Zip
10. It will most likely ask if you want to use the Corrected address
a. Click on the corrected address
b. Click on Continue
11. Click on your car license Plate that you just made
12. Click on Next
13. Review the Information (Permit, Price, Permit Date, and Vehicle)

15. The Resident will have to go on their account and then approve the visitor permit
You just made a Visitor Parking Permit! This permit is only good for 5 nights!

Parking Rules and Regulations:
Parking Rules and Regulations:
1. Parking is permitted only in designated parking areas and stalls within the
community.
2. Each permit holder is entitled to one parking space, as specified in your lease
agreement.
3. Visitors must also obtain a permit account using your visitor authorization code
and must park in designated visitor parking areas only; they are not allowed to
park in resident parking spaces.
4. Vehicles parked in fire lanes, handicap spaces without proper permits, reserved
stalls, or unauthorized areas will be subject to towing at the owner’s expense.
5. Please adhere to the posted speed limits and traffic regulations within the
premises.
Any violations of the parking rules may result in penalties, including booting, towing, fines,
and/or suspension of parking privileges.
